2021 Golden Pride Registration Form and Agreements

All band and color guard students should complete this form which includes the Band Handbook Acknowledgement of Receipt, UIL 8 Hour Rule Acknowledgement, and CISD Permission for Publication Agreement.

Please complete this form to inform the band staff of your summer plans and availability to attend Summer Sectionals, Summer Rehearsals, and Block Parties.

UIL Preparticipation Physical Exam and Medical History Form

All band and color guard members are required to fill out the medical history form and complete a pre-participation physical with a medical doctor this year. Please turn this in to your band director by May 28 (the last day of school).
