PREVIOUS GRADUATES
How to request a transcript:
The request must be in writing by mail or email or form attached below. Please include your full name, year of graduation, date of birth, and address where you would like the transcript mailed. (Include maiden name if applicable.) Mail requests to the address below.
If you prefer to pick up your transcript in the counseling center, please email requests to cwatkins@c-isd.com prior to pick up. Please allow 24-48 hrs to process. Form attached here for a request - CHS Transcript Request
